Mis-prescription of medication

In this case David Gazzard represented an 86 year old lady who suffered from bipolar disorder.  Despite being prescribed her normal medication by her GP she was mistakenly given tablets of twice the strength by her local Pharmacist.

As a consequence she became very ill with lithium toxicity, chronic renal impairment and dehydration and spent ten days in hospital.  Following her discharge from hospital the client suffered a loss of confidence, mobility problems and memory loss.  As a consequence she lost her former independence and relied heavily on family and friends for care on a day to day basis.  Medical reports were obtained from a Geriatrician and a Consultant Psychiatrist and after admitting liability compensation of £30,000.00 was paid by the pharmacy.
